It doesn't copy whether that creature is tapped or untapped, whether it has any counters on it or Auras and Equipment attached to it, or any non-copy effects that have changed its power, toughness, types, color, or so on
It does in fact qualify, though it obviously cant be force-transformed with Tetzin, Gnome Champion or through any other means than melding it with Urza, Lord Protector or Mishra, Claimed by Gix
After a few turns, it transforms into Deathbonnet Hulk, a 3/3 creature that continues to grow by exiling creatures from graveyards, whether youre leveraging your own or hating on your opponents graveyard
If the permanent that returns to the battlefield has any abilities that trigger at the beginning of the end step, those abilities won't trigger that turn
I don't approve of that sort of thing, but it does happen
